The Practice Health Atlas^TM (PHA) is a decision support tool, designed by the Adelaide Western General Practice Network, for General Practitioners (GPs), Practice Managers and other Practice staff. Yorke Peninsula Division of General Practice is pleased to offer you this service in association with AWGPN. The Practice Health Atlas (PHA) aims to inspire general practice teams to reflect on their activities and to develop business models for more effective health care services/outcomes (innovation). It is based on the synthesis of relevant, high quality and timely practice health data, as well as using such data to predict future health care needs and trends (intelligence). What does it mean for my practice? A PHA for your practice can be utilised in at least three ways: As an information resource Compare with the GP / Practice’s existing knowledge For business and clinical systems modelling / re-design Complete discussion points for topics raised in each section Professional Development (RACGP points) Apply for the Active Learning Module through the Division Objectives To create vitality in general practice – a learning organisation that is both participative and reflective through CPD and CQI (e.g.PDSA) To develop a professional culture around quality health data in the general practice setting, with a focus on up to date and complete health summaries (including all co-morbidities) To integrate health data with other relevant data sources (e.g. census and bio-informatics data) - population health informatics and spatial mapping (GIS) To synthesise aggregated de-identified general practice data at Practice and Divisional level (and beyond) to assist with local health needs assessment, planning, and funding To use quality data to reflect on practice clinical and business performance through non-threatening quality peer review processes and self-audit To use quality data to develop business models and drive innovation in general practice in areas such as practice business systems, infrastructure/workforce development, and the effectiveness of health care delivered by health care providers / multidisciplinary teams Outcomes The Practice Health Atlas^TM is an evolving General Practice decision-support tool with multiple potential uses: Data Quality and Management Team-based care Pro-active practice population health care Clinical performance monitoring Reducing health inequalities Business systems development Accreditation Professional education Marketing of Practice Practice Amalgamation With GPs and staff involved to date, it has demonstrated its potential to develop a professional culture around quality health data and as a driver of innovation in health care service delivery.
